How to Join

Signing up is simple. Use the form below to set up your monthly gift by credit card or contact Amber Ebert, Donor Development & Communications Coordinator, at aebert@cghsfoundation.com to set up a donation via credit card or direct deposit.

You will receive one consolidated tax receipt each year for all your monthly gifts — making tax time simple and stress-free.
